Besides daytime hours, our care team is available nights, weekends and holidays.
Hospice provides quality of life and comfort care for hospice eligible patients. Comfort is defined by the patient and family, not the hospice agency. Goals of care are determined by the patient and family and Patient/family goals are our goals.
We believe that all hospice-eligible patient and those who love them deserve the best care at the end of life. An eligible patient does NOT have to:
“Be ready to die”
Sign a DNR (“Do Not Resuscitate”) form
Give up comforting treatment
Have made all of decisions about care goals prior to electing hospice
Give up hope for a cure
Have a caregiver who accepts the patient’s diagnosis
Have a caregiver at the time of admission
Give up the attending physician
